Keep in mind that both cocci and canker are protozoas. I dont know if they look the same under a microscope. Are there any respiratory symptoms? Facial swelling can occur with coryza, MG....but there are other symptoms.
It sounds like she wasnt a healthy chick to begin with. It was probably best that she passed. Sorry for your loss.
Chicken saddles are your best bet, or get rid of your rooster.

I dont know what kind of worm it is. It is NOT a worm that a chicken gets internally. Besides, since you stated that it was moving, internal parasites CANNOT survive outside their host.
