Might be Respiratory disease or nothing...

My polish hen was breathing through her mouth and had discharge, which I noticed when she shook her head and all the stuff was flying out of her nose and mouth, the discharge was thick, a bit white and had hints of yellow. She didn't free range as much as my other two roosters did, spent a lot of time standing still and breathing hard. I set up an "nursing nest" for her. But when I was going to put her in she didn't have anymore discharge and is breathing fine.
Should I be worried? Should I isolate her anyway?
 
It could be any one of a myriad of respiratory problems and its not always easy to diagnose (from my limited experience at least). Eggcessive's advice i agree with but you may wish to consider adding vitamin supplements / electrolytes to her water to help boost her immune system to fight whatever illness she has.
