Have no idea what is going on

Okay so this pullet is supposed to be 5 months old now but she seems so small for her age. She is also very skinny. She eats and drinks I have wormed her she did have a tape worm but this didn't make her any better. She has no symptoms other than standing and closing her eyes all the time then she will snap out of it and walk around then close her eyes again. No runny nose, coughing or sneezing any ideas? I would hate to cull her if there isn't anything wrong or possibly something more I can do. I do put her on chickens starter and she gets vitamins in her water every other day.I have had her since she was 8 weeks old and she has always been like this. Even my rooster gets on to her for sleeping all the time. He doesn't hurt her but dances around her and will fuss until she gets up

What type of wormer did you use? It can make a difference as not all wormers treat all different types of worms.

I would try re-worming her with Valbazen, as it kills all worms.  Here's an informational link:  http://www.backyardchickens.com/t/401475/worming-with-valbazen

She may have coccidiosis. You should try giving her Corid.

If she does and she had it for this long will she ever get fully better? Will she get my other hens sick?
I don't need a carrier as I have 5 baby chicks inside in a brooder and 2 18 weeks old hens coming from a hatchery next month
