Hen mauled by a 6th month old puppy

About 2 weeks ago we caught my puppy trying to play with My Jersey Giant Hen.  When we got him off her I thought her neck was broke so we set her in a nice warm hay area and a few hours later my boyfriend went out to take care of her she was still alive so we brought her in the house still thinking she was gonna die she barely walked  every few steps shed flop down but I was determined she was happy as she would make her way following me around the house to be held making her pretty chirps talking to us, She is now up and walking round eating on her own  at first I had to hand feed her as she didnt move her head up and down to get it herself.  My question is shes back to 100% and needs to be returned outside this is her first Michigan weather house is warm and its been -8 degrees  i dont want to take her out there then herget something from the warm to the cold whats the best way to do it for her safety?

Either wait for a warm day (into the 20's F) or place her in an animal carrier and take it to either enclosed porch or coop. After a couple of hours release her into coop. Make certain she does not get too involved in fights during re-introduction.

Thank You I followed what you said except she didnt stay in the dog crate long but we watched her closely there were no fights or any issues returning her she went in the coop making her chirping sounds and all the other chickens came rushing and clucking back.  It has now been 4 days since she was returned and I am thankful she is still with us and doing much better as far as my puppy he still thinks they are to play with so hes getting a shock collar but only to keep him from jumping on them and for now hes chained and never outside unsupervised off the chain I take him to the pen with me and correct him if he bothers or rushes the flock and hes responding good to that. heres a picture of her and my daughter the day we took her back out.
