I've read all the posts in this article. I understand that the vet is 4 hours away, but it would be best to look up on specialists in the area. If not, I suggest checking on him everyday as much as possible and following advice given from the others. They say the ill heal up faster if there are supporters. This may not be helpful, but I hope your rooster gets better soon. If his toes get amputated, he'll learn to walk again slowly.
