I have two hens that are about 5 months old who have diarrhea and aren't looking as energetic as before. Still scratch and go crazy for food. Drinking plenty of water but seem to be laying down more than usual. I took those two out and monitoring them. Today I noticed one has a terrible looking "vent" and all around area.. Never had this issue before.. Non of those age have started laying..

From what I can tell it looks just inflamed and red but I couldn't see any actual blood. The diarrhea is what's on the feathers but is actually more brownish than it looks in the picture.. But does have a reddish tint..

Give it calcium. It could be coccidiosis or possibly capillary worms.

Maybe look into diatomaceous earth? And add some electrolytes to water to help with possible dehydration issues from diarrhea? Im not an expert though. Lets hope others chime in.

It looks more like vent pecking to me. I would clean up their vents with a warm soak and when dry, put on some Nustock cream or spray them with BluKote. It would be wise to pen them together to heal for a couple of days. Make sure that they have enough room, get outside to range daily, and and are getting a little extra protein in their diets. Put some probiotics in their water also. Here is some info on vent pecking: http://www.thepoultrysite.com/articles/830/cannibalism-by-poultry/
