Comment if I missed one.

You missed skunk, possum and weasel. For me the worst is weasel. In general it's dog. By far the most birds are taken by neighbor's dogs.

I always feel bad when I hear a story about a predator getting a chicken or other bird.

For me it has been neighbor's dogs, possums, and raccoons.  It's a little hard to differentiate between the possums and raccoons without actually witnessing the attack, but we caught a possum in a trap right after one attack so I'm assuming it was him.  He would panic the chickens then one made the mistake of sticking its head through the chicken wire and he bit if off, but couldn't consume much of the bird.  That illustrated the superiority of hardware cloth.

Probably depends on where you live.  Here in the suburbs hawks got all our neighbor's chickens.  Have a friend by a river and raccoons got all of hers.
