Does anyone keep a flock Journal?

I have a book also. I started with all shipped eggs. I kept a record of how many eggs I ordered from each seller and cost after shipping. I figured that into a per egg cost, but after hatch I re-did it as a per chick cost. Plus information on every egg that didn't make it to hatch and why.
I have a separate list of all birds hatch dates, names and descriptions.
 
I keep a journal/ record book for my chickens and for my garden. But for my chickens, I keep track of history and origin, where I got them, breeding, feeding, egg production, etc. Everything I need to know to make a informative breeding program.
