how do i fill out the form?

Just copy it from the front page, post it in the comment, and fill it out like how you see the other forms.

Current teen dad of 17 mixed hens and pullets.  R.I.P. Audrey, our giraffe-necked, torpedo-egg-laying, lady-in-charge RIR: April 1, 2011-June 11, 2013.  You will be missed.  No hen could ever have done a better job as a stand-in rooster; I hope you know that.
