As long as you keep up with the regular cleaning i don't see a problem either. (Now to keep my kids from trying to talk me into something like this too lol)

I agree, I'd be a little tentative trying out the puppy pads because, I too, think that they would most likely eat it. I cant think of anything my chickens didn't try to pick at when given the chance.

Milk crates should work just fine. I know that some people like to attach a small lip on the edge of the crate to hold the bedding in the box (as well as the eggs) but even this shouldn't be too difficult.
I'll have to agree with keeping it tall enough to get into. If it was only a couple feet wide, I can see keeping it low if you were to put a hinged roof on top so you can access the run. Keeping it taller definitely makes cleaning easier as well as enables you to access a sick/injured bird...
