What is the breed and gender of this chicken? Also what is the aprox. Age? I think but not sure I got 1 eggs from her 3 weeks ago and have only had her for 4 weeks??

Red sex link hen who has had a rough life! She looks like she's either been picked on or had mites, something like that. That may be why she's not laying well. As to age, well, at least 6 months. After that, it's really difficult to tell how old a hen is.
