Well are you talking about frequent occurrences? Occasional is not to worry.
 
Small blood spots inside an egg can be caused when hens are frightened by predators or people and the hens go crazy and fly or run into the very same wire that protects them from predators. Children playing, screaming, or running around the run or coop is one way that this can happen. Because it takes a few days for blood spots to be enclosed in eggs, blood spots are not immediately associated with stressful events in a hens' life.
