Chicken has poor balance and yellow diarrhea

Hi there, I have a brown shaver she is approx 15 months old. Few days ago she had a real rustled dirty bum since then that has cleared up but now all she does is lie down and sleep.
If she stands and leans forward she tips over. Still drinking and settled but does not really move.
Any ideas much appreciated
