Chicken lethargic, head shaking and yellow diarrhea

One of my chickens isn’t doing well. I noticed a few days ago she was just standing under a tree and didn’t follow the flock into the coop. I had to go get her and pick her up and put her in the coop. Today I noticed she had yellow foamy diarrhea and just sitting in the coop. My chickens are around 3yrs old. They are my first flock and have been healthy until now. Any suggestions?
 
Can you post some photos of your hen and her poop?

When is the last time she laid an egg?
Have you ever dewormed your hens?

I would check to make sure her crop is emptying overnight, look her over for lice/mites, feel her abdomen below the vent between her legs and take note of any swelling, bloat or fluid.

Sometimes it's difficult to determine what's ailing a hen. Do what you can to get her hydrated, then see if she will eat a little for you. Report back your finding and let us know how she's doing.
 
I checked her abdomen and it feels bloated but soft.

The crop is on the right sight of the breast. When your hens have been eating, you may see a bulge and if you feel of that you'll feel food/water. The crop should feel empty or flat first thing in the morning after a night's sleep. If it isn't, then you would want to address that.

With her abdomen feeling a bloated, she may be having some reproductive issues. I would keep her hydrated and eating. These problems can be common in laying hens.
