Extremely sick chicken

Hey guys, my wife just inhereted 11 hens off her passing nonna. I built a very big pen for them, they have eaten all the grass already, and all the hens seem happy but 1!

She doesnt go inside to roost at night, stays under the pen (still enclosed) doesnt come out at all when we left then free range for atleast 4-5hrs a day.

We put her in a box in the garage pic attached. Her eyes are red, when they are open, which is rare and she barely moves at all. Even when we handle her. Her bottom and very dirty, looks like diahorria.


Ps not sure if it helps, but in the last ten mins has shook her head almost as if she is pecking at her shoulder 3-4 times and slowly lowers back into a hunch

Any suggestions?
