Help with Injured or Sick Chicken!

I found my hen like this today. At first I just noticed the missing feathers on the top of her head and thought she was molting. Then I looked more closely and saw there was a puncture or small cut. There has been a roster coming to our flock so I thought perhaps it was him. But the top of her head and her eye in particular are swollen, her crop is discolored and her inner eye lid isn't opening & closing properly. I suspect it's an injury of some kind, but I suppose it could be disease?? I don't know. Should I separate her from the flock? I don't know how to treat her as well. Any immediate help is appreciated!

Sound like an injury of some kind but either way, it will be best to separate her. Wish I could give more information on how to treat but without more information I am at a loss.

thanks, do they usually heal on their own? she's isolating herself, but still eating and active.
