Help with NPIP Certification in Michigan

Does anyone have information on the documents needed for NPIP certification in Michigan? About how long does certification take? How much can I expect to pay for it (our flock is about 30-40 birds)? I've contacted the state vet here, but I had to leave a message and am waiting for them to call back. I can't seem to find any information on it online.

I'm a Certified Salmonella Pullorum Tester in the State of Michigan. I only test on behalf of a club that I belong to that sponsors a few fur and feather swaps and raises money for 4-H. I have heard that some people will charge about $0.75 per bird plus travel.

Both my husband (Iamfivewire) and myself are certified NPIP testing agents for pullorum. Usual cost is $1.00 per bird and if you live far enough away, we may add bit on for gas to travel.
