pipped eggs not hatching

What would be some causes for pipped eggs not hatching? Also when I check the eggs that failed to hatch or pipped there is a fully formed chick in there that died some time after the 18 day. What could be the causes for this. I tried to keep a close eye on the humidity in the incubator and when I went into lock down the humidity reading inside there was 60%.

These might help you determine what went wrong if you open and inspect your unhatched eggs. My first guess is that your humidity may have been a little low, but it could possibly be something else.

Those are useful links. I have only done 2 hatches but also had several fully formed chicks that died at ~day18 or later, most without pipping at all. I also followed a dry incubation method. If I do this again, I may try for more traditional humidity levels.
