I have an approximately 5 month old Bantam hen who is clearly sick. She has been very lethargic and slow for the last two weeks. It wasn't until three days ago I noticed one side of her nose is swollen. She keeps her eyes closed most of the day, and barely eats or drinks. Today she hasn't eaten at all. I try to hold her and let her rest as much as I can. Her comb is also very pale, which I've read can indicate illness. What could be wrong? None of my other chickens are sick.

Just guessing here but that looks like some type of infection. What I have done in the past if a hen has been ill for several days and I don't know what is wrong is start them on antibiotics hoping that it is an infection of some type and this will save them. If the hen is not eating or drinking I use an injectable otherwise I sprinkle it on their food. Hopefully others will have an answer.
