Sick Chicken

My chicken has been sick for a week and 4 days. Has its head tucked down face 1st on the ground is very weak. I have bought electrolites and vitamin packets to add to water and Vitamin E she will barely eat or drink. I dont know what to do! Can anyone help me i dont know whats wrong with her.
