Thanks to all that replied. Turned out that it was a prolapse. We isolated her, cleaned her up and started on the prep-h right away. We kept her in an isolation cage, in the garage and kept it as dark as possible, hoping she'd stop laying and give herself a break.
She refused... laying one...
I see in other threads that we should try to "push this back in". It's kind of stiff or hard. I don't want to hurt the hen. Should we be pushing it if its hard?
Well, I brought her in and cleaned her up a bit and this is what it looks like now.

I went out to gather eggs today and found one with a LOT of blood smear on it. I check the hens and one has a vent that looks pretty nasty. This is the first I've noticed it but we were low on eggs the past few days, so I don't know if she layed any the past few days prior to this one...
