How do you tell if an egg is fertile?

You can't tell an egg is fertile by candling it. Only after some development does candling work. You can tell by cracking it open and looking for that bull's eye.

Has the egg been incubated at all, or is it fresh?

There is no really reliable way to tell if an egg is fertile unless you incubate it. An incubated fertile egg will develop blood vessels, and eventually, a chick. The egg in your photo doesn't appear fertile, though it can be hard to tell fertility from a photo--the camera changes the lighting, etc.
 
If you have a rooster and he has been mating the hens for at least 2 weeks, then you can assume most of your eggs are fertile. Set a few more than you need in case some don't hatch or you run into other issues.
