Please, I really need help how to treat this bumblefoot. He had a surgery and remove two plugs but his feet still swelling. I'm not sure what to do next. By looking at the pictures do you think I need to do the second surgery? Or any suggestion

There are likely more corns inside the foot pad. It doesn't look like you need to cut it open more but soak it and really squeeze it to get them out. A large infection like that can have several corns in it.

Thank you, taprock

He is very suffering due to this bumblefoot. Not easy feeling at all when I have to do that again...
Do you think I have to give him any medication at this point?

If anyone have any suggestion please... I need help...

Here are two I've had to deal with in the past. After cleaning and squeezing the foot put a antibacterial salve in it, just not with pain reliever in it. You may have to repeat several times. As the swelling goes down and it heals it is easier to see/feel individual corns that need to come out. Good luck!
