Oh, gosh. So sorry to hear you had to pay $215 for that fly larva.

A rabbit I had as a kid got one of those years ago. My mother, farm-raised, knew how to handle it right away. A pair of tweezers and a strong, strong tug. End of worm. A little antiseptic and the rabbit was fine. No need to sew it up. Did take a long time for the hole to heal though.

At the vet hospital I worked at we used to use chicken (or other meat) baby food, warmed in the microwave just a bit. Sometimes if kitties can't smell they won't eat. Any stinky food warmed up would be the same - fancy feast salmon for instance.
You should take him to the vet. At 10 yrs old there are alot of things that can be wrong but he's still got some years left! Kidney, diabetes, worms, etc. Contact your local shelter or rescue group and see if they cannot offer some advice/help.
