Chicken with Respiratory Problems

I could really use some advice or suggestions. The vet wasn't much help and so far nothing I've tried has made much of a difference.

-Cochin, 14 weeks old, weighs 4 1/2 pounds (has gotten bonier than other chickens of same age)
-Rasping breathing with intermittent coughing, also some gurgling sounds, breathing is worse when stressed.
-Has had symptoms for about 6 weeks. After symptoms first occurred they have not improved or worsened. Only change has been the weight loss.
-No other birds are exhibiting symptoms.
-No signs of injury or trauma.
-Started with cough and then rest of symptoms started, nothing changed that might have caused them.
-Only eating and drinking when separated from others. Will eat crumbles but not pellets. Will also eat meal worms and millet.
-Poops looks normal.
-VetRx,tried Tylan once per day for 3 days per vet. Noticed slight improvement on 2nd day but then got worse again. Found other information saying to give Tylan for 5 days so tried that but no change. Also tried Oxine treatments in cool mist vaporizer 3 times a day for 10 days with no change.
-Tried taking to vet and vet was unable to diagnose. I would really just like to find out what is wrong with her and get her better. I'm fine with taking her back to vet but apparently the vet will need some direction. Going to the vet also stressed her out and made breathing worse for several hours.
-Wood frame chicken house, closed at night but access to fenced pen during the day and house is open then. Bedding is pine shavings cleaned every day. Prior to pine shavings I was using straw but switched to the shavings about 8 weeks ago.
 
So since treatments for respiratory diseases haven't worked and she doesn't sound to have other symptoms like a runny nose or eye bubbles, I'm going to suggest that it might be gapeworm. I'd try treating for that and see what happens. Treat all the birds just in case. Worst case scenario she doesn't improve and you've wormed your birds. Can't hurt to at least try.
