The answer is:


I just finished a safety course required by work and this was one of the slides they showed.

The woman is the owner of the bar which has a basement accessed by a trapdoor used for product storage.

One of the employees opened the trapdoor without notifing the others and she turned and 'went down the hatch'

She suffered a broken leg among other injuries.
