I went out to my hens to hang their feed and noticed tiny worm-like creatures in the sand. What could they be?!

It's really hard to see from you picture but they could be mealworms or maggots of a sort, eating the chicken food off the ground. I get them sometimes around the bricks that support the waterer. The girls love them.

It's impossible to tell from the pictures, but I wonder if it's a Lesser Mealworm (Alphitobius diaperinus)? Lesser mealworms are commonly associated with stored grain products and poultry. They can cause considerable damage as they burrow into wood seeking a place to pupate. Most importantly, they are an important vector of several poultry diseases.
