is the survey in my email from BYC for real?

I got an email that says it is from BYC:

Is this for real? I am wanting to make sure before I go to any website and fill out anything. I know scamers can make up emails that look all too true, so am cautious.

Yes, this is a legitimate email from one of our sponsors, but there seems to be a problem with it that we're looking into very closely.
