Record Keeping

Do any of you know of a source of some type of record Keeping forms or have created your own system ? My system of jotting things down on scraps of paper has got to be changed. I'm wanting a way of keeping up with vaccination, worming, medication. Also show records, sales etc. Any help would be appreciated.

I have a little note book, it has the names ( if the chickens have them) the year they were born, the breed, colour legring number and colour and any notes about the animals. at the back I have my notes on when I powdered, wormed medicated etc. I also did this for my mothers sheep, but in that I had their name, year born, mother father, breed , colour genetics and any notes. a good well made note book is a good idea, as it might get battered around a bit
