people often put herbs in nest boxes to prevent pests like worms, mites, lice, ect. in their flock. Curtains are optional, but recommended. (especially if the nest boxes are in the run instead of in their own little secluded room.) I would say that your 16 week old date is good, just in case...

you mentioned it was small. could be a fairy egg. sometimes hens lay very small eggs often with no yolk, and it's called a fairy egg. it's perfectly normal, especially for new layers. hope this helps :)

Her cere does look some what abnormal... While it could be that, i would say to look into brooder pneumonia, as symptoms seem to match.

could be beginning stages of bumble foot. Keep an eye on it, and if it gets worse, spreads to the bottom, and or you see a scab, treat it as soon as possible.
afterwards. pretty much her whole life, though i cant really remember her doing it as a chick. but as soon as i put her and the other hens in an outside coop for the first time, (about 3 months of age) she started to do it.
