It may have been originally caused from another chicken pecking her. Feather lose like that is unlikely to grow back till her next molt. So keeping it covered for protection till she grows them back is a good idea if it's from pecking. Unless you do discover lice or mites.

Is she enclosed to a run or do they get to free range? I'm more curious about what's in the feather surrounding the bald spot. What do you use for bedding in the coop and nesting boxes?
Have you done a full body check over? Feel for any unusual lumps and bumps? Mites/Lice? Poop check?

Is she still being bullied? Chicken combs don't grow back. Depending on the severity of the injury if some comb is left it will heal. Infection is the biggest concern with head injury because it's so close to the brain. Is the injury healed? After a month it should be pretty much healed.

At Day 20 I wouldn't worry. Lack of movement is normal take a lot of energy preparing to hatch and not much room left in there. You'll probably wake up to chicks. Please share pictures of the new babies.
