australorp loosing colour on its face

I'm a newbie and not sure what is going on with my hen. Her face has been gradually losing its colour and it seems to be growing white "bits" on it too around her face and on the crest. Would she have mites or a fungus and if so how can it be treated. How often should you worm chooks and why wouldn't a young hen lay egges?
