I've de-broodied many hens successfully in the past, but this one hen is beyond me. Everything has failed, so yesterday we put regular chicken wire in the dog crate and a 4" distance of the ground. We initially put in a 4x4 piece of wood, but then she just squats on that; it seems she would still get warmth from the 4x4. We took it out this morning, and now she's just standing around on the chicken wire. If I were a hen, I wouldn't be too happy. So... if the consensus is that the wire it's hurting her, we'll put the 4x4 back in, but will this "fix" her?

If she's a particularly large hen the wire might be an issue, but a smaller to average sized hen should be fine.

I feel your pain, 3 of my 5 hens have tried to go broody on me (not all qt once, thankfully). The dog crate with a 2x4 for perching worked beautifully for 2 of them, but that 3rd hen was determined to sit on bare wire! Eventually I moved the broody buster out of the coop into the secure run so that she couldn't even see the nest. That finally did the trick! Within a day she was done with being broody.
