How far apart should the slats on a chicken ramp be placed? and how wide should they be?

There is no magic distance. It would also depend on what type of bird you have, if you will have young birds in the coop. The slats are only for traction. Mine are about 8" apart and I have young birds and bantams that use my coop.

Our ramp is roughly 48" long and 13" wide we spaced ours about 8" apart and they are each about 1 1/2" wide.
