Chick died at hatch break out

Hi, I'm using a new incubator. We had 28 healthy hatches out of 46 viable eggs (identified from candling). One died when it broke out of the shell, it appears. It happened overnight so I didn't see it. I discovered the deceased chick in the morning. It zipped out fine though so I'm wondering if there's any insight into what happened. I referenced the incubation guide but I'm not exactly sure what it falls under? Yolk not absorbed? We also hatched two with curled feet, one with one spradle leg, one zipped all the way and never hatched (I thought it was dead until I heard it peeping when removing from incubator). The one that zipped around and didn't hatch, I helped it out but it doesn't seem to be able to control its body. It took a day to start moving its head but it has never stood up. These are in addition to the 28 healthy chicks. We bought 66 of the eggs at an auction so there's no telling how long they were stored, at what temperature, the health or age of the parent flock etc. 24 of the eggs came from our flock but we didn't separate which ones. Out of the 90, only 46 looked promising on day 16. From day 0-3, the eggs were in an incubator that wouldn't get above 97⁰. Day 4 they all went to our newly procured GQF 1270. The one that zipped but didn't hatch zipped on day 20 and was the first to pip and zip. Everyone else hatched at the end of day 20 and during day 21. The one in question who hatched and died hatched at the end of day 21. I'm attaching the pictures I took after removing from incubator. It stayed in the incubator about a day postmortem as I waited for the pipped eggs to hatch. All help is appreciated.

it was most likely low humidity. The humidity is usually around 60 to 70 RH but if the humidity was at that i would say possibly to much humidity during incubation or not enough. The perfect humidity during incubation is around 50 to 55 RH. Hope that helped, I'm by no means an expert however i have done a few other hatches.
 
An update: the chick I helped out started running (not walking) everywhere. We call her Forest. She had wry neck badly. We've been giving vitamin E and b vitamins on an egg at least 5 times per week for the past 4 weeks. She's almost 100% better. The wry neck is so subtle at this point, I don't think you would be able to tell if you didn't have prior knowledge. We going to continue the supplement and are considering putting her with the rest of the flock. She stays separated with two spradle leg chicks. We've successfully treated the spradle leg and curled toe chicks as well. One is still in shackles but at least 3 have recovered fully.
