chickens die out of no where!

Please help! My chickens die off one by one slowly over a period of time and I'm unsure why. I feed them, along with they are free range. I deworm them. I give them nutrients and or save a chick in their water once a week. Or every other week. I had a red hen die lasted week. And this morning had a 4 month old chick die out of no where. The chicks are separated from the adult flock. Has food and water. Is there something in my soil? What am I doing wrong?
 
What do they look like when you find them dead?

I think its more likely something is visiting your coop and offing them. If you have rats, minks or weasels in your area, they can slip or dig their way in without leaving much evidence.

Usually if they're sick, they'll show some kind of sign before they kick off (usually when its almost too late to save them).

My red hen was sick. She was gurgling it sounded like. She didn't last through the night. The baby chick I didn't notice anything. And the chick I have now is sick. I don't understand how they're getting sick? I bought the chicks from tractor supply and I had them inside my house under a heat lamp for a little over a month. Then I slowly transitioned them to be outside. They've been outside comeplety for 2 weeks now. And 1 has died and one is on its way out.
