Chicks dying right before hatch/after internal pip

This is my second batch of chicks that have died right before hatch. There was about 15 eggs in both hatches, however both times the majority of them weren't my eggs which made me feel better. Barred bantam cochins. The first hatch I didnt have much hope for, 15 out of 36 eggs started developing, when I moved them to the hatcher there was only about 9 still moving and only one of them actually hatched. The lady told me after I was already incubating them that she had refrigerated them which was unheard of for me. All three of my chicks I incubated with this batch made it.

The hatch I just had, were supposed to hatch yesterday. Out of 16 eggs, 14 started growing, 12 made it to the hatcher, only one hatched a day early. Along with my 2 cochins. Last night when I checked 6 had internally pipped, 6 had died already. This morning there were 3 more dead and 3 internally pipped still. These eggs were from one of my chicken friends in town. Good quality silkies.

I know I had a batch of quail do this before.. but I had a rotten egg in the bunch. There were no rotten eggs in this bunch. And I have 3 separate incubators I split my eggs in incase one malfunctions, all of my eggs dont get ruined
 
I know how disappointing it is to have chicks in the homestretch that fail to hatch. :hugsThere are a number of reasons they quit before hatching or die shortly after hatching - this resource may shed some light on why.

I actually had a very good hatch with refrigerated eggs last year, but that's not optimal, and the best hatches happen under optimal conditions. What kind of incubator are you using, and what temperature and humidity? Autoturner? The more detail, the more helpful it may be to troubleshooting.
 
I have three different incubators for constant incubation. Ones a little giant, and two are farm innovators. I keep the temperature at 99.5. There are auto turners. I have hatched probably hundreds of chicks at this point but these two hatches are concerning me. I dont mess with humidity in these, I'm in Missouri. In the hatcher, a gqf I believe, I keep humidity at about 60 ish temp is still around 99.5 Once they hatch and start rolling eggs around they move into another farm innovator to dry off. I've lost a few eggs due to chicks rolling the egg over and the pip is facing down.
