Elderly Hen

I have a 13 year old hen (the only one left from my original flock!) She is very easy going and sweet. Her eyes are cloudy and she seems to be going blind, and her feet have some arthritis. I notice her getting picked on by others sometimes and I am wondering if it would be a better idea to separate her. I don't want to stress her out by taking her from the nesting box where she has spent most of her time over the last several years, but I don't want her injured either. Any advice would be helpful. Thank you!
