extreme molt around head and neck slow to grow back

one out of my chickens, a Plymouth Rock, lost just about all of her feathers around her head and neck leaving her bald. She also has molted all around her body, not looking bald but very thinned out. The neck and head feathers have come back pretty well but not fully and she looks extremely thin now like she has lost weight. Her comb is also very pail. Is this normal or is there something wrong with her?

I would suggest you treat her for mites/lice as they can cause symptoms like these, have you wormed your chickens?
