My hen is 4-5yrs old, an RIR bantam, still lays almost every day unless moulting.
She's always been a skinny bird, but recently I noticed she's extra thin even for her.
She still acts normally and is top of the pecking order. They are regularly wormed (half the reason why I noticed she was thin because I weigh them when worming them). The other two hens are 2yrs and 1yr old and they're still a good weight. They eat layer pellets, some scraps and free range.

I'm wondering whether the nutrtional needs change as they get older. Her eggs did go a little soft for a while but we sorted that out, they still look slightly balooned though. I thought of perhaps buying some broiler food to feed her in the morning when they first come out would help her put on some weight?

Try offering Oyster Shells all the time for her, they will eat what they need is what I have been told. I keep a feeder with Oyster shells and another with Grit in the coop, I add some once a month maybe.

The above post must have changed since I added my above advice, Not sure what to say for adding weight.
