How old is your hen?
Is that a hard shelled egg?
Is that membrane or poop on and around the egg?

Could be a glitch, if it only happens once...could be something more serious.
I'd keep a close eye on her.......any other symptoms of illness?
Is she eating, drinking, pooping normally...is she active?
 
She's almost 2, it is a regular egg under whatever it is, it's membrane and she has a wound on her neck that is slowing her down.
 
It's the white part of it is soft and kinda squishy. I've never seen any thing like it!
 
That looks like tissue and good bit of it....hopefully it's not tissue from the lining of the reproductive tract.

It could be another egg membrane, fully shelled eggs sometimes migrate back upstream from the shell gland and join another yolk, albumen, membrane and shell.....
.....or just some of these....producing an egg within an egg.

Does the inside of the shelled egg look normal?

I'd keep a close eye on her, take a look at her vent, and hope for the best.
