1) What type of bird , age and weight. Polish, 2 months old, weight unknown
2) What is the behavior, exactly.Neck bend Look Like the head is tucked under chest
3) Is there any bleeding, injury, broken bones or other sign of trauma. No Blood broken bones don't know
4) What happened, if anything that you know of, that may have caused the situation.Nope nothing, They ( 6 polish) are in a 12x5 coop and have a fenced in run
5) What has the bird been eating and drinking, if at all. Not sure if he has been eating or drinking. They are fed Kent chick go. And have water with acv ever day.
6) How does the poop look? Normal? Bloody? Runny? etc. Haven't noted and odd poops in their coop.
7) What has been the treatment you have administered so far?Just found him like that, took him out the coop and have him in a hutch.
8 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet? Right now I am wondering if I should put the poo thing out of it's misery. If anyone think there is a chance it will heal with a neck wrap or something I'll try it but a trip to the vet is out.

I haven't a clue what happened to this chick. I only can think sick or it fell off a roost or hit it's head.
