Help! I have a very sick chicken!

So I've talked about my 4 year old tetra brown, mable in the past. But today I have become extremely worried about her condition. She's been having this labored breathing, she has more trouble during very humid days. She is very lathargic at times and has a limp in her right leg. Today I noticed she's been having green fiecies. I have had her dewormed with safegaurd equine dewormer about three weeks back. I need help for treatment. I've heard these chickens can love for up to ten years. She's a good chicken and one of my head hens if my flock. Please help!
 
Can you look at her lower abdomen and check to see if it is enlarged or swollen? Ascites or a reproductive infection or problem might be causing her to have labored breathing. You should repeat the worm dosage at 10-21 days, so I would give another dose. Does she lay eggs anymore? Is she overweight, normal, or underweight? Can you see any evidence of injury in the leg (redness, green bruising, or swelling?) Is she eating and drinking? How does her crop feel?

She is underweight. She eats she's not completely off of food yet. What should I do for treatment. I'm limited on what medicines I can get.
 
She isn't laying as much as she used to. She sometime has spells of her twitching her head,.like she has something in her throat that is preventing her from breathing. She doesn't gape like it's gaperworm. But it's kinda like when they get something in their nose, like water
Did you look at her lower belly for any enlargement that could be ascites (water belly or a reproductive problem? ) Is she laying eggs?
