Help with 2 month old chicken

I had a chicken get sick within hours it puffed up andtuck it's head into its chest and it start to rain last night and just stood in the rain and didn't move so I went to pick it up and it just lay there and let me pick it up so I put it in the sick box last nig under the hear lamp and the puffyNess went down but the cHicken wouldn't move and would keep its eyes closed so I wormed it and thence tonight it still layed in the cage and when I pick it up to check on it it seems like it hurts when I pick it up and touch it the chest is still swallow still please help !!!
