I have a hen that has been off balance for the past 2days and now she has started just falling over and just laying there and she's only 4 or 5 months old, any suggestions on what key be causing this?

Can you post some pictures of her? I'd also like you to check her eyes and say how they look. color, pupil. Also need to know what they are getting for feed and any other suppliments you may be giving them.

She eats shelled corn and we started giving her the antibiotic that u Put in their water and her eyes are fine everything looks normal she seems to be doing a little better but is still off balance a little

My first year having chickens I had a hen (buff orpy) do the same thing. The other chickens took advantage of her lack or coordination and started beating the crap out of her. I had no idea what it was. It was just like she had a horrible weakness in her legs and could barely stand. (wasn't laying yet) Eating and drinking and pooing normally. I brought her inside in a large dog crate with food and water, and on a surface with traction. I'd let her out before and after work and made her walk around. After about two weeks I wast able to put her back in the coop. Soooo, no answer for you, but with some patience she may be fine.

She needs more than shelled corn. You need to give her some kind of processed feed or you will need to supplement with vitamins, all of the time. Corn only provids carbohydrates. They need a whole lot more to stay healthy. Since you have started an antibiotic, you will need to continue it for 5 days at least, whether she needs it or not. Change her feed first. Get her and the others a balanced feed from a farm store or Tractor Supply. Free ranging your birds, while you are there to supervise, will help as well. Corn doesn't supply all that a chicken needs to survive. I would feel better if you were giving her a wild bird mix rather than all corn. Corn is the equivelent of sugar in our diets. It tastes so good, but it doesn't do any good.

They all are getting vitamins supply put in thier feed so I'm good with that and they are free range chickens they have a coop they can go in but they can come out whenever they want and she seems to be getting better ( or I pray she is ) .
