How much safe-guard pellets do I give?

I have a flock of five and need to know how much of the safe-guard .5% pellets to give them and the best method of giving it.
 
thank you, i just tried to get my hen with symptoms to eat one pellet mixed in with some yummy meal worms and she totally refused to eat it. So, it isn't gonna work. the reason i tried the safeguard pellets was because someone suggested it, which i believe that it works for her chickens, but i don't see it happening with mine.

I have albendazole liquid that I gave her (one hen in particular that keeps relapsing) a couple months ago, which perked her up a little bit, then I tried Equimax paste, that finally seemed to bring her back to normal. However, upon renewed symptoms, I just gave her a dose of the albendazole two days ago, which should have perked her up a bit, as it did before, but it doesn't seem to be having much, if any affect. Maybe I'm dealing with a tape worm??? I don't know. the other hens don't seem to be catching it other than one whom i dewormed at first sign and she is doing well now. There are five hens altogether. I know everyone says i should deworm all of them together, which I did originally, but honestly I cant see continuous deworming all the hens when only one seems really affected. I'd love others' thoughts. I'm learning and trying really hard to keep them healthy and alive.

Ok, What symptoms are you seeing?
Why do you think it's worms?
 
She exhibits the same symptoms as before when I gave wormer and it seemed to bring her back to health. Symptoms- weak, thin, not eating much (not even treats), standing alone on one or the other leg while constantly resting the other and closing her eyes during inopportune times, feathers appear scrappy, comb shrunk, loose droppings of basically normal color. Another observation: crop does have food in it that feels like grit or sand (but she hasn't been outside the run which has pine shavings for bedding- there is no sand, and grit in dish looks untouched).
 
Post some pictures of her droppings. Tapeworm segments are normally seen in the droppings moving around. See the video below for tapeworms. Tapeworms can be treated with Equimax, but other worms are treated with the albendazole with 2 doses 10-14 days apart. Is she laying eggs, and has she been molting recently?
