laying hen lacking head feathers

Ok, I am clueless here. Need some help finding out what might be causing this problem.

I have 1 hen that I thought was in molt due to lack of feathers on head. But I caught her laying and this condition has lasted now over a 2 months. It is NOT mite or lice. No other hen is showing the problem. All eat a 20%protien diet and free range all day from sunrise to sunset. Could this be a medical condition or is it just the other hens pulling them out on the perch? My only other thought is she has an allergy to something growing that she rubs against outside. She is acting totally normal and has been eating well. So any thought about what might be going on with her?
 
do you have a rooster? my hens look like this too but its cause the roo will hold on during "THE" deed and pull on the feathers
