Listless 2-year old hen

First time having a a sick hen. Yesterday, noticed she was just standing next to the water can when all the others had gone to roost. Today, same thing. She was under the coop. I picked her up and put her on the roost. Seemed very weak, and unstable. A couple of minutes later, she let loose with the most foul-smelling stream of diarrhea.

Any suggestions? How can I help her?

Worm her as well as the rest of your birds and visually inspect her for lice/mites, especially around the vent area.
