KMR mixed up normally or what ever replacer you can get but add a crushed cat/kitten multivitamin to each bottle. Make sure the vitamin is totally powder (I run mine through a spice grinder) and make the hole in the nipple a little bit larger. If they are younger than a week old I usually mix in a little bit of live cultured yogurt as well. Most bottle baby kittens thrive on this, in fact I've only lost one in recent years and that was because of a respiratory illness.
ETA, I use this mix until 4 weeks old then I start pureeing canned cat food to mix with the plain formula 50/50, then in a week it's in a bowl, not pureed with the formula... usually in another week they are totally off formula
