Malnourished chicken?

I think I have a problem with one of my chickens. . .
We have about 20 chickens, but one is suffering and I think it is because she is not eating the layers pellets, only the corn we put out. She looks lethargic, doesn't run about like the others and draws her neck into herself. We feed her scraps when she is apart from the others but how can I help her? Do I need to change the feed or take her to the vet? I am inexperienced with chickens so I need some advice! Thanks.
 
How much corn are you feeding? Treats and table scraps should only comprise 10-15% of a bird's diet.

Sounds more like a health issue to me. I've seen hens who starved to death with no other symptoms before, who other than slowing down (as in literally, walking and moving slower but with otherwise with the same behavior) did not in any way appear sick, until you lifted them and realized a bird who should've weigh 4 pounds weighed just a little over 1. On the other hand, a particularly sick bird tends to not eat, while also displaying symptoms like ruffling and lethargy.
